How To Submit A Case
Davis and Associates will accept your case submission in a variety of methods. You can contact the office to schedule a local pick up, you may ship your package via UPS contact the office for a shipping label or submit digitally via Itero Scans or Primescan.

Download and print one form for each case you are wanting to submit. Fill out all of the information that is required.
Choose Shipping/Delivery Method
If you want to submit your case via UPS, we offer free shipping both ways. Pack up your case with all the necessary items, then request a label and pickup time.
Submit digital scans via iTero, Medit, Primescans or Trios
